Dear Parent/Guardian,
Last year your child in year 10 should have received their DTP (Diptheria, Tetanus, Polio) and MENINGITIS ACWY vaccinations, HOWEVER if your child did not receive their vaccinations last year.
Please go to https://yny.schoolvaccination.uk/dtp/2023/northyorkshire and complete a consent form.
PLEASE ONLY COMPLETE A CONSENT FORM IF YOU ARE SURE YOUR CHILD DID NOT RECEIVE THESE VACCINATIONS LAST YEAR, IF YOU ARE NOT SURE IF YOUR CHILD RECEIVED THEM PLEASE CONFIRM THIS WITH YOUR GP.
Once you have completed a consent form, your child will either receive their vaccinations in school, or we will invite you to attend an out of school clinic.
